Agreement to Terms

By accessing or using the services of DENTIST CA, DENTAL PRACTICE OF JOHN K. LEUNG D.D.S. P.C. ("we," "our," or "the Practice"), you agree to be bound by these Terms and Conditions. If you do not agree with any part of these terms, please do not use our services or website.

Dental Services

We provide dental services at our location at 500 Primrose Road, #3, Burlingame, CA 94010. Our services include but are not limited to:

General dentistry (examinations, cleanings, fillings)

Appointments and Scheduling

Appointments can be scheduled online, by phone, or by email.
We require at least 24 hours notice for appointment cancellations or rescheduling.
Repeated late cancellations or no-shows may result in cancellation fees or termination of the patient relationship.
We reserve the right to refuse service to anyone who consistently fails to keep appointments.
Please arrive 15 minutes before your scheduled appointment time to complete necessary paperwork.

Patient Responsibilities

As a patient, you agree to:

Follow recommended treatment plans and post-care instructions

Outstanding balances may be subject to collection procedures.
